Supreme Health Care, Inc. recognizes that as a continuum of the acute care process, the patient/client may require supportive care in the home environment. Supreme Health Care,Inc. will accomplish this objective by, but not limited to, the following:
Providing nursing personnel base upon the acuity level of care as certified by the attending physician.
Ensuring the continuity of care through qualified care practitioners that are knowledgeable on all care modalities as required by the client.
Maintaining and documenting direct communication with the attending physician relevant to standards of care.
Establishing and maintaining active patient client case conferences on all active care cases.
Ensuring the confidentiality of client medical records.
Timely completion and submittal of all financial billing relevant to primary and secondary reimbursements.
Ensure compliance to all regulatory guidelines and laws as established by governing regulatory agencies.
